Registration

To sign up for a club at Fairhaven,

  • Log into Final Forms with your registered school email.
  • Once logged on, under “Sports and Activities” across from your student’s name, select “Add Sports and Activities.”
  • Under the column, “Join Extracurricular Activities,” find the club (FMS Club:) that you would like your student to participate in.
  • Select the box, then select “Update Sports and Activities.”

Once these are complete you can exit out of Final Forms.  If a student is just participating in only clubs at Fairhaven a sports physical is not required. If you feel that the advisor needs to know about your student’s health history, please reach out to the advisor separately.
